Colonoscopy prep has a completion problem

25%
of colonoscopies involve inadequate bowel preparation
36%
of patients cite taste as a major barrier to finishing prep
~69%
of adenomas missed when bowel preparation is inadequate

Most patients go through colonoscopy prep with a paper instruction sheet and no structured support. The result? Confusion about timing, difficulty with taste, and lack of guidance are the primary reasons people don't complete their prep properly — leading to cancelled procedures, repeat visits, and missed diagnoses.

Clariva was built to fix this.

Flavor Support for PEG 3350 — In Development

The problem

PEG 3350 (commonly prescribed as MiraLAX) is the most widely used colonoscopy prep base. Patients mix it with Gatorade or water and drink large volumes over several hours. The taste is consistently cited as one of the biggest reasons patients struggle to finish their prep — up to 36% report taste as a significant negative impact.

The solution

Clariva is developing custom-formulated flavor stick packs designed specifically for PEG 3350 prep solutions.

How it works

Tear open a stick pack, pour it into your PEG 3350 prep mix, stir, and drink. The natural flavors and masking agents help reduce the salty, chemical taste that makes prep so difficult to finish. No measuring, no guesswork — just better taste.

Key features

Natural flavors — no artificial ingredients

Zero calories — won't interfere with your prep

No red, purple, or blue colorants — these colors can mimic blood or mask the colon lining during your procedure, so they must be avoided

Designed to mask the salty, chemical taste of PEG 3350

Single-serve stick packs — easy to use, no measuring

Currently in formulation development with flavor house partners. Sign up to be the first to know when they're available.

The flavor stick packs are a food product designed for taste support. They are not a medication, supplement, or medical device. They do not replace or modify your prescribed preparation. Always follow your healthcare provider's instructions.

For Healthcare Practices

Improve prep compliance. Reduce cancellations. Support your patients.

Clariva is a free-to-implement companion app and product platform that helps your patients complete their colonoscopy prep — reducing no-shows, lowering prep-related call volume, and improving the patient experience.

What sets Clariva apart

Unlike paper instruction sheets, Clariva provides timed, personalized digital guidance that adapts to each patient's procedure date and prescribed protocol — with the comfort products to match. Patients receive step-by-step reminders via SMS and email so they always know what to do next, and they can order curated prep kits delivered to their door. It's structured support for the prep window, not just another document to hand out.

The hidden cost of inadequate prep

Every GI practice deals with the same problem: patients who show up inadequately prepped, or don't show up at all. The result is cancelled procedures, repeat scheduling, lost revenue, and frustrated staff. Meanwhile, your team fields calls from confused patients who can't figure out when to start drinking, what they can eat, or whether they're doing it right.

The root cause isn't patient negligence — it's a lack of structured support during the prep window. Patients get a paper instruction sheet and are left to figure it out alone.

Top barriers to adequate prep

Confusing timing and instructions

Taste and volume of prep solutions

No structured guidance during the prep window

Dietary restrictions patients don't understand

Breaking down the numbers

200 colonoscopies/month at a typical practice
× 25% involve inadequate bowel prep
= ~50 affected procedures per month
× $2,000 average revenue per procedure (facility + physician + anesthesia)

= $100K/mo at risk from inadequate prep — lost revenue, wasted suite time, idle anesthesia staff, and slots you can't backfill last-minute

Guided digital prep programs and curated kits directly address the root causes of inadequate prep — helping patients comply, reducing confusion, and keeping your schedule on track.

Estimates based on published literature. Actual figures vary by practice.

How Clariva works for your practice

Developed by physicians

Reduce procedure cancellations

Inadequate prep doesn't just mean a rebooking — it means lost procedure revenue (~$2,000+), wasted suite time, idle anesthesia staff, and a slot you typically can't backfill last-minute. Clariva's guided digital prep with timed reminders addresses the root cause. Published research shows SMS-based guidance cut inadequate prep rates in half.

Lower prep-related call volume

Prep-related calls are one of the top uses of staff phone time. Clariva's automated reminders with dietary guidance, medication alerts, and step-by-step instructions answer patient questions before they pick up the phone — freeing your back-office staff for higher-value work.

Better prep quality saves lives

Better bowel preparation means better visualization during the procedure — which means better detection of polyps and early-stage cancers. Inadequate prep leads to missed adenomas and delayed diagnoses. Improving prep quality directly improves patient outcomes.

Curated kits reduce patient confusion

Patients often buy the wrong products or scramble at the last minute. Clariva's curated prep kits ensure they have exactly the right items — shipped to their door — eliminating "did I get the right thing?" calls to your office and reducing day-of prep failures.

Zero cost to your practice

Clariva is free to implement. There are no licensing fees, integration costs, or per-patient charges. Revenue comes from optional product sales to patients, not from your practice.

Prep compliance insights (coming soon)

Track prep compliance rates across your patient population. Identify patterns, measure improvement, and demonstrate quality metrics. Provider dashboard in development.

Built by physicians, for physicians and patients

Clariva was developed by a team that includes a practicing physician and a board-certified gastroenterologist who have seen firsthand how patients struggle with colonoscopy prep. We built Clariva because we knew there had to be a better way to support patients through the prep process — and to help practices reduce the costly downstream effects of inadequate preparation.

Science & Research

The evidence behind better prep

A growing body of published research supports what Clariva is built on: structured digital guidance improves bowel preparation quality, better preparation improves detection, and better detection saves lives.

Statistics on inadequate bowel preparation rates, why it matters, and the consequences for patients and practices

Up to 25% of colonoscopies involve inadequate prep

Studies estimate that up to one in four colonoscopies involve inadequate bowel preparation. The most commonly cited barriers include taste, volume, timing confusion, and lack of structured guidance — all of which are addressable through better design and digital support.

Inadequate prep leads to cancelled procedures, shortened surveillance intervals, repeat visits, and missed diagnoses.

What changes during prep

Bowel preparation can reduce microbiota diversity and shift microbial populations immediately after cleansing 1.

These changes are often transient, with recovery trending toward baseline over days to about two weeks for many people 2.

Screening reduces CRC mortality by up to 62%

A meta-analysis of 29 studies found that colonoscopy screening was associated with up to a 62% reduction in colorectal cancer mortality. The effectiveness of screening depends on the quality of the examination, which is directly tied to bowel preparation quality.

Why prep quality matters for screening

When preparation is inadequate, the colonoscopy may need to be repeated sooner, adding cost, inconvenience, and additional prep cycles for the patient. Adequate preparation on the first attempt supports complete examination and appropriate follow-up intervals.

Research on taste and palatability as a compliance factor and the role of hydration in preparation success

Up to 36% of patients cite taste as a significant negative impact

Research shows that taste is consistently one of the most commonly reported barriers to completing bowel preparation. Up to 36% of patients report it as a significant negative impact during the prep process.

Difficulty with taste leads to incomplete consumption of the prep solution, which directly reduces bowel cleanliness and procedure effectiveness.

Why hydration matters during prep

Prep involves large fluid shifts. Hydration supports comfort and can help people stay on schedule. If you have fluid restrictions, follow your provider instructions.

  • Helps reduce fatigue and headaches for some people
  • Supports tolerance and completion of the full prep volume
  • Pairs naturally with better taste support

The connection: taste, completion, and outcomes

The research supports a clear link: patients who struggle with taste are less likely to complete the full prep volume, incomplete prep reduces bowel cleanliness, and poor cleanliness leads to missed lesions and repeat procedures. Addressing taste is not just about comfort — it's about clinical outcomes.

The core evidence: SMS reminders, smartphone apps, and mHealth meta-analyses showing improved preparation quality

A growing body of research shows that structured digital guidance, including SMS reminders, smartphone apps, and timed notifications, can meaningfully improve bowel preparation quality and patient compliance.

SMS reminders reduce inadequate prep (PERICLES-II)

In the PERICLES-II trial, patients who received SMS-based preparation reminders had significantly lower rates of insufficient bowel preparation: 9% versus 19% in the control group 3.

Mean bowel preparation quality scores were also significantly higher in the SMS group, with improvement across all colon segments.

Digital navigation cuts no-shows

A randomized trial of 1,625 patients found that SMS-based digital navigation reduced colonoscopy no-show rates from 14% to 8% and improved adequate bowel preparation from 88% to 93% for screening colonoscopies 4.

The SMS messages provided timed instructions on dietary modifications and preparation steps.

Smartphone apps improve preparation scores (van der Zander)

A randomized controlled trial found that patients using a personalized smartphone app achieved higher bowel preparation scores, particularly in the harder-to-clean right colon 5.

Adequate preparation rates reached 99% in the app group compared to 95% in controls.

mHealth technologies and bowel cleanliness (El Bizri)

A systematic review and meta-analysis of 10 randomized trials encompassing 3,383 patients found that mobile health interventions, including apps, SMS, and video guidance, significantly improved bowel preparation quality scores 6.

Diet adherence and patient satisfaction were also higher in intervention groups.

Technology-based instructions improve compliance by 20% (Wonggom)

A systematic review of 47 randomized controlled trials found that technology-based bowel preparation instructions, including mobile apps, SMS, video, and web-based education, improved adherence to preparation instructions by 20% compared to standard care 7.

Emerging research also explores how behavioral science frameworks can be applied to digital colonoscopy screening interventions. A registered clinical trial (ISRCTN14407050) examines the use of digital nudging based on the MINDSPACE behavioral framework to improve colonoscopy screening adherence.

Reinforced education improves preparation quality (Guo)

A meta-analysis of 18 randomized trials with 6,536 patients found that reinforced patient education, including structured guidance and reminders, improved adequate bowel preparation from 74% to 87% 10.

Diet compliance also increased from 83% to 91%, and both adenoma and polyp detection rates improved significantly.

How preparation quality directly impacts adenoma detection rates, missed lesions, and the chain from prep quality to detection to outcomes

The quality of bowel preparation directly affects what can be seen during colonoscopy. Research consistently shows that better preparation leads to higher detection of adenomas and polyps, while poor preparation leads to missed lesions.

Poor prep reduces adenoma detection by up to 47%

A meta-analysis of 27 studies covering over 247,000 colonoscopies found that inadequate bowel preparation was associated with a 47% reduction in early adenoma detection and a 26% reduction in advanced adenoma detection 8.

This means nearly half of early-stage growths may be missed when preparation is insufficient.

Inadequate prep misses up to 69% of adenomas

A prospective multicenter study examined patients who had inadequate bowel preparation and underwent early repeat colonoscopy. The per-adenoma miss rate was 68.9%, with a 60.2% miss rate for advanced adenomas 9.

Poorly cleaned colon segments harbored the majority of missed advanced adenomas and all missed cancers in the study.

Enhanced education boosts adenoma detection by 37%

A meta-analysis of randomized trials found that enhanced patient education was associated with a 37% increase in adenoma detection rate and a 19% increase in polyp detection rate 11.

Sessile serrated adenoma detection, which is particularly important for cancer prevention, also increased significantly.

The connection between guidance, prep, and outcomes

The research supports a clear chain: structured digital guidance improves preparation quality 357, better preparation increases adenoma and polyp detection 81011, and more complete detection supports more effective screening and cancer prevention. Clariva is designed to support the first link in that chain.

Frequently Asked

Questions & Answers

Everything you need to know about Clariva, prep support, and how we work with patients and clinics.

For Patients

What does colonoscopy prep actually involve?

The day before your colonoscopy, you drink a prescribed solution that clears out your colon so the doctor can see clearly during the procedure. You also follow a restricted diet (usually clear liquids only) and stay close to a bathroom. The process typically starts the afternoon or evening before your procedure and continues into the early morning of the procedure day.

How does the Clariva app help?

The Clariva app builds a personalized prep timeline based on your procedure date and your doctor's prescribed protocol. It sends you timed SMS and email reminders for each step — when to change your diet, when to start drinking your prep solution, when to take medications — so you always know exactly what to do and when. The app is completely free.

What's in the prep kits?

Our Prep Comfort Kit includes hydration support (Pedialyte), flavor variety for clear liquids (Crystal Light Lemonade), comfort items (Tucks pads, Desitin, flushable wipes), warm liquid options (Herb-Ox bouillon), and a Clariva-branded shaker bottle with measurement lines. The PEG Prep Essentials Kit includes everything in the Comfort Kit plus the OTC medications required for PEG-based prep protocols (MiraLAX, Dulcolax, Gas-X).

What are the flavor stick packs?

We're developing custom-formulated flavor stick packs designed specifically to improve the taste of PEG 3350 (MiraLAX) prep solutions. They use natural flavors, zero calories, and no red, purple, or blue colorants (which can interfere with colonoscopy visualization). They are currently in development and not yet available. Sign up on our to be notified when they launch.

Can I use Clariva with any prep my doctor prescribed?

Yes. The Clariva app supports the most commonly prescribed preparation protocols, including SUTAB, SUPREP, Clenpiq, Suflave, PEG/MiraLAX, Plenvu, and EZ2GO. Our products are designed to complement, not replace, whatever your doctor has prescribed. Always follow your doctor's instructions first.

How do I get the Clariva app?

The Clariva app is provided through participating healthcare practices. When your doctor's office schedules your colonoscopy, they will provide you with access to the app as part of your pre-procedure materials. If your clinic doesn't currently offer Clariva, you can and we'll reach out to them.

Is the app free?

Yes. The Clariva companion app is completely free for patients. It is provided through participating healthcare practices. There are no hidden fees or in-app charges for the app itself. Our prep kits and comfort products are sold separately and are optional.

Is my data private?

Yes. Clariva takes your privacy seriously. We collect only the information needed to personalize your prep experience — your procedure date, prescribed protocol, and notification preferences. We never sell your personal data to third parties and we do not access your medical records. You can read our full for details.

What if I have questions during my prep?

For medical questions about your preparation, always contact your healthcare provider directly. For questions about using the Clariva app or our products, you can reach us through our or email us at support@getclariva.com. If you are experiencing a medical emergency, call 911 immediately.

Is any of this covered by insurance?

The Clariva companion app is completely free. Our prep support products are purchased separately and are not currently covered by insurance. However, colonoscopy screening itself is covered by most insurance plans as a preventive service at no out-of-pocket cost.

Does Clariva replace my doctor's instructions?

No. Clariva is designed to help you follow your doctor's instructions more effectively — not replace them. The app adapts to your specific prescribed protocol and timing. Always defer to your healthcare provider for medical guidance.

How bad is the taste of prep, really?

Most people find the taste challenging. It's often described as salty, bitter, or chemical-tasting, and you have to drink a large volume. This is one of the biggest reasons patients struggle to finish. Our curated kits include flavor options for clear liquids, and our flavor stick packs (coming soon) are specifically designed to address the taste of PEG 3350 solutions.

For Healthcare Practices

How does Clariva work with our practice?

Clariva integrates into your existing pre-procedure workflow. You provide patients access to the free companion app as part of their prep materials. The app handles timed reminders, dietary guidance, and step-by-step instructions based on your prescribed protocols. There is no EMR integration required.

What does it cost our practice?

Nothing. Clariva is free to implement for healthcare practices. There are no licensing fees, per-patient charges, or integration costs. Revenue comes from optional product purchases by patients, not from your practice.

How do patients get access to the app?

Your practice provides patients with access to the Clariva app as part of your standard pre-procedure materials. The specific implementation depends on your workflow — we work with you to find the simplest integration point.

What prep protocols does the app support?

The app currently supports SUTAB, SUPREP, Clenpiq, Suflave, PEG/MiraLAX (including EZ2GO), and Plenvu. We're continuously adding new protocols. If you use a protocol not listed, contact us and we'll prioritize adding it.

Can we customize the protocols for our practice?

Yes. During onboarding, we configure the app to support your specific prescribed preparations, timing preferences, and any practice-specific instructions. If your practice uses a protocol we haven't yet added, we'll work with you to build it. The goal is to match your clinical workflow exactly, not force you into a one-size-fits-all approach.

Does Clariva provide medical advice?

No. Clariva provides general informational guidance, prep reminders, and product support. It does not provide medical advice, diagnosis, or treatment. The app guides patients through the preparation instructions prescribed by their healthcare provider. All medical decisions remain with the treating physician.

How do you handle patient data and privacy?

Our Story

About Clariva

Created from firsthand experience and a simple question: why has colonoscopy preparation not improved in decades?

I have gone through multiple colonoscopies before the age of 30. Each time, the hardest part was not the procedure. It was the preparation. The taste, the volume, the timing, the uncertainty, and the disruption to daily life felt the same every time.

After my fourth colonoscopy, I stopped waiting for someone else to fix it. I come from a family with deep roots in gastroenterology, so I understood both the clinical side and the patient side. The gap between them was obvious: the medical community knows prep compliance is a problem, and patients know the experience is miserable, but nothing in between had meaningfully improved in decades.

The problems are specific: the taste makes people quit early, the timing is confusing, the instructions vary by clinic and protocol, and there is no structured support for the person actually going through it. Clariva was built to address each of those problems directly.

We're improving the colonoscopy prep experience through a combination of digital guidance — a free companion app that builds personalized prep timelines with timed reminders — and thoughtfully designed products, including curated comfort kits available now and custom-formulated flavor packs in development. We work with gastroenterology practices to provide their patients with structured prep support, at no cost to the practice.
